Suitability to pursue the professional activity, including requirements relating to enrolment on professional or trade registers

List and brief description of conditions:  As per QW1- Suitability Questionnaire (restricted) June 2013. and including:
3.4c List of Services of Similar Nature provided over the past 7 Years [Maximum 50 marks]
Applicants must provide details (including start and finish dates, project details, contract sum and references of 3 Building Construction Projects similar in nature and complexity to the services required for this project carried out in a satisfactory manner and meeting the requirements below.
The information requested is attached to this questionnaire at Appendix 3.4c.
All 3 projects must have been completed (i.e. at Substantial completion) within the last 7 years. The Contract value of each project must be between a half and 5 times of the Turnover Level Requirement as set out in the Contract Notice for this project. A maximum of 15 Marks are allocated per project listed and an additional maximum of 5 marks for at least one example of a Primary or Post-Primary School project (these marks awarded in proportion to the marks awarded for the school project itself).
Projects of a similar nature are defined as single building projects (not necessarily educational) with similar construction and service requirements.
For example a shop fit-out or an advance factory is not a similar project, whereas an extension and refurbishment to a Health Centre or Library is. If the project being tendered for is a re-roofing project, the experience needed may be either roofing contracts or complete building projects (It is not necessary that the roofing element is at least half the turnover requirement). Likewise if the project is a replacement of a heating installation, a mechanical contract (whether as main or sub-contractor) is adequate experience whereas an Electrical installation contract is not.
Where in the combined judgement of the Contracting Authority [C.A.] and the references quoted (or otherwise checked by the C.A.) a project was not satisfactorily completed, the Contracting Authority may exclude the tenderer from further consideration.
ASSESSMENT CRITERIA
This Suitability Assessment Questionnaire and Declaration use the same criteria as the Office of Government Procurement (OGP) Suitability Assessment Questionnaire for Works Contractors QW1 with criteria not applicable omitted. The same numbering system has been retained.
The minimum standards for participation in this competition (Suitability Assessment Criteria) are set out below under each criterion. Evidence of compliance for Pass/Fail criteria is not required at Expression of Interest stage.
By signing this Questionnaire and Declaration, tenderers are declaring that they meet the relevant minimum standards in all respects. It must be signed in ink by a Director or Principal of the firm and dated.
Applicants may also satisfy this requirement by submitting an ESPD completed in accordance with EU Regulation 2016/7. By completing Part IV (α) of the ESPD titled: Global Indication for all Selection Criteria, applicants will be taken to declare that they meet the standards set for all criteria where the response is covered by the Declaration at the end of this document or by the standard Declaration at 3.1 below. Supporting documentation should be provided promptly upon request from the Contracting Authority.
