The HSE in Mayo are seeking to appoint a Provider to work closely with the
Community Action on Dementia - Mayo consortium (CADM) for a period of one
year. The Providers Service Team will support the CADM in working with people
with Dementia and their families. The CADM project aims to provide a system of
service provision for people with dementia in Mayo that is person-centred and
based within the person's own community. The Service Team will also aid in
integrating the work of the project with established mainstream services and with
the HSE service plan and Mayo County Council, LCDC Health & Wellbeing
Working Group. They will link directly with HSE Service Teams and local
Voluntary and for profit organisations.
