For the above mentioned works and as part of our Tender Stage 1 -  Pre-Qualification Suitability Assessment, we are providing the following Documentation:

·         1634_Document Issue Register – 180-187 Townsend St_TD Stage 1

·         1634_QW1-Suitability Assessment Questionnaire (& all associated supplements & appendices.)

·         1634_PA-001_Site Location Plan

We invite Candidates to register their interest in obtaining a place on a shortlist for Invitation to Tender by filling out the provided Form QW1 – Suitability Assessment Questionnaire, along with all other Supplements & Appendices as listed below.

 

Full List of Documentation to be submitted:

Completed Suitability Questionnaire Form QW1, including the following completed declarations & evidence:

a)    Self Declarations at Appendices A1, D, E & F or ESPD (where applicable)

b)    Template evidence documents at Appendices B1 & B2

c)     Third Party Certification in the form of declarations at Appendiix A

d)    Evidence of enrolment on a relevant professional Trade Register, as per requirements of Section 3.2 of Suitability Questionnaire QW1.

e)    Evidence of Education & Professional Qualifications (Management) as per requirements of Section 3.4a of Suitability Questionnaire QW1.

f)      Evidence of Education & Professional Qualifications (Personnel) as per requirements of Section 3.4b of Suitability Questionnaire QW1.

g)    Details of average annual manpower & managerial staff as per requirements of Section 3.4e

 

 Completed Health and Safety Supplement 3.4.1 with the following completed declarations:

a)    Self Declarations at Appendices C1, C3, E & G or ESPD (where applicable)

b)    Template evidence documents at Appendices B1 & B2

c)     Third Party Certification to OHSAS 18001, SafeTCert or equivalent

 

·         Completed Health and Safety Supplement 3.4.2 with the following completed declarations:

a)    Self Declarations at Appendices C1, C4, E & H or ESPD (where applicable)

b)    Template evidence documents at Appendices B1 & B3

c)     Third Party Certification to OHSAS 18001, SafeTCert or equivalent

 

The completed documentation should comprise 2 no. hard copies plus 1 no. electronic copy (CD or USB Stick), be clearly marked as “1634 Tender Stage 1 – Pre-Qualification” and submitted no later than January 9th 2019 to the following address:

Please note that where the response to a Criteria is ‘Declaration Required’, evidence will be sought from successful applicants prior to final shortlisting. The evidence should be submitted no later than 10 days after applicants have been notified of the decision to place them on the shortlist.
