I enclose herewith tender forms for the Supply, Delivery, Installation, Commissioning, Maintaining & Staff Training of Surgical Laser Equipment, Accessories & Consumables. Initial requirements inGalway University Hospital and Sligo Regional Hospital (detailed specification attached). This procurement process is being conducted by HSE Procurement, Portfolio & Category Management,
Unit 1St. Colm‘s Centre, Finisklin Business Park, Sligo.
The HSE reserves the right without advance notice (and without giving reasons, unless so required) to amend or otherwise change the process or to terminate the process.
This tender document does not represent a commitment to enter into any contract.
All contracts are awarded in accordance with HSE’s Conditions of Contract and the conditions.
It is imperative that you complete the tenderer statement (attached) and return by email on receipt of Tender forms, as this is also an acceptance by your company of the HSE’s Communications Protocol, Standard Conditions of Contract http://www.hse.ie/eng/about/Procurement/TermsConditions/ and any additional terms detailed in the Request for Tender document and pertinent to the above competition identified in this tender document.
To ensure that HSE is operating in accordance with the Irish Data Protection Act and to identify any risks or possible contraventions of the Act, it is a requirement that your organisation returns a signed copy of the attached document (appendix 5).
The Irish Data Protection Commissioner is insistent that this Service Provider Confidentiality Agreement ver 1.4. is to be signed by all service providers to the HSE.
If you have submitted a signed version of this document previously by means of a tendering process or direct communication with HSE, then please supply a copy of same.
