1.0      Minimum Standards and Qualification Required

 

Refer to the requirements and criterion in the Suitability Assessment Questionnaire and associated supplements and appendices attached to this Contract Notice.

 

 

2.0      Pre-Qualification Criteria

 

The Restricted Procedure in accordance with the Directive 2004/18/EC as transposed into Irish Law by Statutory Instrument 329 of 2006 shall be the procedure governing this competition.

 

Following receipt of expressions of interest, candidates will be assessed and pre-qualified based on the criteria as outlined in the “Suitability Assessment Questionnaire for Service Providers”.

 

Only information contained in the questionnaire and the prescribed appendices, in the prescribed format, will be considered as valid information for the pre-qualification process. Additional introductory or prefaced material not contained within the body of the questionnaire will not be considered.

 

It is the responsibility of each applicant wishing to participate in this competition that they submit a completed “Suitability Assessment Questionnaire for Service Providers” document complying with the requirements of Kildare County Council. Where a group, joint venture, or consortium submits a proposal, a lead consultant must be nominated who shall be fully responsible for providing the services required.

 

Please note that where a group of service providers is awarded the contract, they must form a single legal identity, with joint and several liability and with a designated lead provider. The successful Candidate must submit evidence of legal formation of entity prior to appointment

 

The documentation to be returned must include:

·     A completed Suitability Assessment Questionnaire

The following attached completed declarations:

·     Self Declarations (i.e. A1, D, F, E)

·     Third Party Certification Certificates (i.e. Declaration at Appendix A)

·     Self Declaration Certificates (i.e. B1, B2)

so as to demonstrate/confirm compliance with the relevant criteria.

·     Supplement 3.4.1 with declaration (as appropriate) at Appendix G, or SafeTCert, or OHSAS 18001, or equivalent; also Self Declaration Certificates B1 & B2 in relation to Health and Safety and Self Declaration F, C1, C3, E.

·     Supplement 3.4.2 with declaration (as appropriate) at Appendix H, or SafeTCert, or OHSAS 18001, or equivalent; also Self Declaration Certificates B1 & B3 in relation to Health and Safety and Self Declaration F, C1, C4, E.

·     Supplement 3.4.3 with Self Declaration Certificates B1 & B3 in relation to Health and Safety and Self Declaration F, C2, C5, E.

·     Supplement 3.4.4 with Self Declaration Certificates B1 & B3 in relation to Health and Safety and Self Declaration F, C2, C6, E.

Applicants failing to meet with the selection criteria will not be further considered for short-listing. Applicants will be short-listed on the basis of information submitted.

 

 

3.0      Submission of Suitability Assessment Questionnaire

 

The Suitability Assessment submissions should be delivered by registered post, recorded delivery service or by hand so as to arrive not later that the date and time given in the Contract Notice. Applicants for suitability must return two (2) copies, and three (3) electronic portable document format (PDF) copies on compact disc (CD), of the completed Suitability Assessment Questionnaire and Appendices, in a sealed envelope, clearly marked with:

 

"Tender Suitability Assessment Application to Tender for Upper Liffey Valley Regional Sewerage Scheme Extension to Osberstown Waste Water Treatment Plant - Stage 3 Contract No. 1 - Waste Water Treatment Plant"

4.0      Additional Information

 

Tenders are advised that Kildare County Council is subject to the requirement of the Freedom of Information Act, 1997. If a Tenderer considers that any of the information supplied with his/her submission is either commercially sensitive or confidential in nature, this should be highlighted and the reasons for its sensitivity specified. In such a case, the relevant material will, in response under the Act, be examined in light of the exemptions provided in the Act.

 

Prior to appointment the successful Applicant will be required to produce a current Tax Clearance Certificate of C2 Certificate, from the Republic of Ireland Revenue Commissioners. In the case of a non-resident Applicant, he/she should be in a position to produce a statement (in lieu of a Tax Clearance Certificate) from the Republic of Ireland Revenue Commissioners that he\she has satisfied them as to his\her suitability on tax grounds to be awarded the contract.

 

Each Applicant will be required to provide evidence that he\she meets the requirements of a Registered Employment Agreement or provide evidence that he\she operates a satisfactory pension and sick pay system
