The intention is to form a panel of providers for a Community Ophthalmic Service for each of the following lots: Lot 1: Children aged 0 to 3 years and 11 months Lot 2: Children aged 4 years to 7 years and 11 months; Lot 3: Children aged 8 years to 15 years and 11 months.
