The aim of the project is to develop a national framework for person-centred planning in disability services in Ireland. Person-centred planning tools encourage “good planning leading to positive changes in people’s lives and services”. The purpose of person-centred planning is to discover how a person wants to live their life and what is required to make that possible. The person with disabilities is central to this process. It is expected that an agreed national framework for person-centred planning will support disability services to consistently achieve good practice in person- centred planning, and therefore support the achievement of positive outcomes for people who use those services.
