The Health Service Executive, Dublin North East would like to invite you to submit a Suitability Assessment Questionnaire in respect of the above and accordingly enclose the following documentation for your attention:
1. Suitability Assessment Questionnaire
The Health Service Executive intends renovating the building known as Blackwater House on the St. Davnets Campus Roosky, Monaghan Co. Monaghan to deliver Primary Care Services including G.P services to serve the population of North Monaghan.
Works will involve the extensive upgrade and refurbishment of Blackwater House to provide a New Primary Care Centre incorporating 2 no. GP practices, a Dental Treatment Facility, HSE Treatment & Therapy facilities, Bookable Consultation Rooms, Administration Facilities and associated works at St Davnet's Hospital
SAQ to be submitted on etenders.ie
Responsibility for the submission of SAQ prior to the Response Deadline rests solely with the Contractor.
Site visit not necessary at this stage.
