Suitability Assessment Questionnaire - Pre-Qualification for selection
as Works Contractor and as PSCS
Dear Sir/Madam,
You are hereby invited to prepare and submit documentation for the above process in accordance with the SAQ (QW1) and attached appendices. Reserved Specialists will be appointed for the Mechanical, Electrical & Lift elements of the works.
Return of Submissions
The closing date for receipt of submissions is 12th November, 2018. The Completed documentation and accompanying details must be delivered into the custody of the Office of Public Works, Project Management Services, Unit 20 Lakeside Retail Park, Claremorris, Co. Mayo by not later than 12.00 noon on that date. Submissions received after such time and date shall be rejected. Time is of essence in this regard. One hard copy and one copy on electronic disk to be supplied.

- Identify your company on the disk.
Suitability
Please note that the QW1 assessment criteria is a combination of Pass/Fail and Qualitative.
